I am a navigator who lives in and currently serves your community. I have completed advance care planning training and education through the University of Colorado Anschutz Medical Campus. I would like to work with your organization to hold a one-hour group educational session (free of charge) in a comfortable setting of your choice at a time convenient for people involved with your organization. I will provide all of the materials and will lead an interactive discussion about advance care planning. During the one-hour session, the following things will be discussed: 1. We will review frequently asked questions about advance care planning. 2. Group members can discuss their understanding and concerns about advance care planning, including examples of both positive and difficult experiences. 3. Members will chose and document a preferred health care agent if unable to speak for themselves. 4. We will discuss the question: “What would your health care agent need to know about your values and care preference to make the best decisions for your health care?” 5. If participant’s wish, they will document their preferences for their personal health care wishes. 6. I will provide support in the process and be available should questions arise.
